I think, if you were asking anything about the client, you would take the 7th. And then if it were about the client's health or ill-health, you would count from their 7th (turn the chart). So you'd look at (1st from 7th or 7th house (same house) for their general well being and health and if asking bout illness, the 6th from their 7th or major illness, 12th from their 7th.

Many Astrologers draw up a chart for the time of the reading - the moment when the client walks through the door and note the Ascendant Sign and Ruler placement.

Unless the Client requests the Astrologer to comment on her health then it would be considered 'none of his business' and he is has no entitlement to ask.
